To apply for initial PSP network access and installation or for changes to your PSP service:
Make as many copies of the application as you need.
Use a single application form for multiple access lines at the same location, if they are for the same type of service or, clearly denote different types of lines and number of each needed on one application (at the same location). Use a separate application for each location.
Use the same form for all service additions, changes or removals.
If you are a Verizon customer, send a Blanket Agency Agreement if you have not done so.
Mail, fax or e-mail (NPCC@verizon.com) the completed application to Verizon. Be sure to specify the equipment location for 9-1-1 service and to include the Blanket Agency Agreement.
We process completed applications in the order in which we receive them and honor the first application we receive for a given location. We reserve the right to deny an application if a site owner demonstrates that he or she has revoked the applicant's authority to provide PSP service at that location.
Verizon date-stamps all applications when we receive them. Please remember that an incomplete application or submission of forms other than Verizon required form(s) will result in delayed order processing (for example, excluding state and docket numbers and/or PUC certificate, where required, will delay your order).
