How to claim your one-time credit:

You must fax or email any invoice from your previous broadband provider within 30 days of placing your Verizon High Speed Internet order. Fax to: 877-875-8843 or email an attachment or scanned soft copy to: broadband.switch@verizon.com

The invoice from your previous broadband provider must show:

• Same customer name as your new Verizon High Speed Internet Account
• Same customer address as your new Verizon High Speed Internet Account
• Previous broadband provider company name
• Active broadband service on or after Jan. 1, 2008

All other details, such as account number, payment method, etc. should be crossed out prior to faxing.
