Feature Group A (FGA) Form
Overview
The Feature Group A (FGA) form is used to provide end user circuit details for new installations, changes to existing services or disconnects.
Based on the type of activity indicated on the access service request (ASR), not all forms may be used for ordering. Related forms for a FGA ASR include Multi EC and Testing Service Request.
Description
The FGA form is sectioned to provide specific information related to the end user services being requested:
- Administrative - basic account information
- Circuit Detail - specific information related to product being ordered
- Service Options - based on product being ordered
- Remarks - optional free form field for use to provide further information
