Subject: FCC Triennial Review Order

Effective May 3, 2004, CLECs that have signed a Triennial Review Order (TRO) amendment (or an interconnection agreement that implements the terms of the TRO) must populate two new required fields (the "CNO Field" and the "QA Field", as described below) on Access Service Requests (ASRs) when ordering or converting to certain unbundled products. In addition, CLECs must also place certain information in the "Remarks" field when ordering, converting to, or re-certifying EELs, EEL M-loops (i.e., commingled UNE Loops), and commingled UNE Transport, all as further described below. Pursuant to the terms of the TRO, Verizon will not provision the subject products if such fields are not populated. This letter only applies to UNEs and UNE combinations ordered via ASRs. In Verizon West, DS1 and DS3 Loops are ordered via the LSR. A separate notice will be provided regarding LSR ordering practices.

CNO Field
CLECs must populate the CNO (Case Number) Field when submitting ASRs for the following unbundled products: DS1 and DS3 Loops, EELs, UNE Transport, and Dark Fiber. The CNO field identifies whether the CLEC has signed an amendment to the TRO or an interconnection agreement that implements the terms of the TRO. An ASR with a CNO entry of "Y" indicates that the request should be processed pursuant to the CLEC's contract reflecting TRO terms.

QA Field
CLECs must populate the QA Field when ordering the following unbundled products: DS1 and DS3 Loops, EELs, and UNE Transport pursuant to a TRO amendment or an interconnection agreement that implements the terms of the TRO. When the CLEC submits an ASR for a UNE or UNE combination, to anticipate existing facilities are not available, the CLEC can request that Verizon proceed automatically with the required routine network modifications by populating the QA (Quote Authorization) field on the original ASR with a "Y". Please note, for Dark Fiber, a CLEC request for Routine Network Modifications is submitted via the Dark Fiber Inquiry Process, not the ASR process.

As an alternative, if the CLEC chooses not to populate the QA field on its original ASR with a "Y" and existing facilities are not currently available, the request will be queried by Verizon and typically will include the reason why facilities are not available. At this point, the CLEC may request that Verizon proceed with the required routine network modifications, provided the CLEC agrees to pay Verizon for such facility modifications. In order to request that Verizon perform such modifications, the CLEC must respond with a supplement to Verizon's query by populating the QA field with an entry of "Y".

A "Y" in the QA field constitutes the CLEC's agreement to pay Verizon's standard rates for routine network modifications. Verizon will process the ASR and provide the CLEC with a Due Date for the request when facilities will be available. Please note, except for Dark Fiber, Verizon will not provide a price quote for such facility modifications. The rates for facility modifications are provided to CLECs as part of the TRO amendment (or in the price schedule to a new interconnection agreement implementing the terms of the TRO).



REMARKS Field on Transport Form
When ordering, converting to, or re-certifying an EEL, EEL M-Loop (i.e., commingled UNE Loop), or commingled UNE Transport circuit pursuant to the TRO, the CLEC will be required to provide Verizon with the following information:

1. Local telephone number assigned to each DS1 or DS1 equivalent circuit: Due to space limitations in the REMARKS field on the Transport form on the current ASR, only one local telephone number, instead of 28, will be required for each DS3 EEL, EEL M-Loop, or commingled UNE Transport circuit ordered. (However, the foregoing does not affect the CLEC's obligation to comply with the TRO and to maintain complete and correct, auditable records in accordance with the requirements of the TRO.)
2. Date each local telephone number was established by the CLEC in the 911/E911 database.
3. Collocation Real Estate Billing Account Number (including Customer Code) that shows collocation was purchased pursuant to 251(c)(6).
4. For DS1 EELs, DS1 EEL M-Loops, or DS1 commingled UNE Transport circuits: The DS0 local interconnection trunk ID that serves each DS1 or DS1 equivalent circuit in the same LATA as the end user premises served by the circuit. Note that the CLEC must transmit the calling party's number in connection with calls exchanged over the trunks on the interconnection facility. Due to space limitations in the REMARKS field on the Transport form on the current ASR, this information is required for DS1 circuits only. (However, the foregoing does not affect the CLEC's obligation to comply with the TRO and to maintain complete and correct, auditable records in accordance with the requirements of the TRO.)
5. For DS3 EELS, DS3 EEL M-loops, or DS3 commingled UNE Transport: The DS1 interconnection facility ID associated with the above DS0 local interconnection trunk ID that serves each DS1 or DS1 equivalent circuit. There must be one DS1 interconnection facility ID per every 24 DS1 or DS1 equivalent circuits. Due to space limitations in the REMARKS field on the Transport form on the current ASR, only one DS1 interconnection facility ID is required. (However, the foregoing does not affect the CLEC's obligation to comply with the TRO and to maintain complete and correct, auditable records in accordance with the requirements of the TRO.)
6. CLLI code of the CLEC's Local Switch that serves each DS1 or DS1 equivalent circuit. The switch must either be registered in the LERG as a Class 5 switch or must be able to switch local voice traffic.

Please note that this information must be provided in the REMARKS field on the Transport Form of the ASR until new fields are obtained pursuant to OBF guidelines. Each item of information must be separated from the next with a semi-colon (;). A sample appears below. For any TRO eligible orders received prior to this change, this information will be processed manually.
