    Overview

    • Not all locations will require a site survey. Verizon will determine applicability of a site survey.
    • Should a site survey be required, a Verizon representative will make contact with the Local Contact (LCON) or Alternate Local Contact provided on the Service Request to schedule the site visit. The LCON must be familiar with the site and the service required and be able to provide all required access. To ensure that this critical step takes place as quickly as possible, LCON contact information must be accurate and an alternate LCON (ALCON) should be provided on the Service Request.
    • If the Verizon representative determines that the work required will involve common building space (e.g., telecommunications riser closets), he/she will request that the LCON/ALCON notify building management and arrange for the required access.
    • During the site survey, the Verizon representative will assess the site to determine what is required from the customer in order for Verizon to provide the requested service. The details will be made available in the End User Contingency Requirements form (EUCR), or in an email summary of the completed site survey outlining the pathway, space, power and environmental requirements that must be provided by the customer prior to delivery of the requested service. These requirements may include providing Verizon with written authorizations for items such as Right of Way and collocation arrangements.


    EUCR

    • The End User Contingency Requirements (EUCR) form provides the customer with a convenient, itemized list of activities that must be performed by the customer in order for Verizon to provide the requested service, as well as order information, contact information for both parties, and the date the LCON has committed to completing all items. This form is a very effective tool for ensuring that the required items are completed on time and that committed in-service dates are met. The EUCR form is automatically sent to you by Verizon (via email). Alternative forms may be utilized depending on conditions and the work to be performed at your location.
    • Proper Customer Space Readiness is crucial to timely provisioning. Please pay special attention to the itemized list of site readiness requirements. Space, power and conduit are the responsibility of the customer and must be provided in compliance with Verizon specifications.

    eWPTS

    The Wholesale Provisioning Tracking System (eWPTS) Status Tool will provide Wholesale customers a Worklist View of their orders where Verizon has identified End User Contingency Requirements (activities the end user needs to complete before the service can be provisioned).

    You can access the eWPTS Status Tool via the CSG Accessing Ordering Application .

    Once logged in, go to the Request Tracking Menu and select the eWPTS Link.

Wholesale customers should issue a Supplemental order changing the Request Due Date to the date they will be ready.
