For a Lease Arrangement (LA), the following fields will be used:

LA - Lease Arrangement
LADATED - Date of Lease Arrangement
LANM - Lease Authorization Name.

These fields are found on the Access Service Request (ASR) and must be completed at the time services are ordered. The Access Service Ordering Guide (ASOG) contains the industry’s ASR forms and fields that you will need to place your orders.
