Collocation Qualification Process - COE Installation
     
 
Who Does it... What Happens...
Collocator or Collocator Installation Company
  • Completes and submits Verizon West Collocation Application form (colocapp.doc) to Verizon West Wholesale Markets.
Verizon West Wholesale Markets Account Manager
  • Reviews Verizon West Collocation Application form for completeness.
  • Checks form to see who has been identified to perform equipment installation; Collocator or other equipment installation company.
  • Verifies if Collocator or their equipment installation company is on Verizon WestApproved Contractors list or Collocator Qualified Installation Company list.
  • If not listed, sends Collocator Application and other required documents for Collocator Installation Services to HQ Network Services-COEI Support.
Collocator or Collocator Installation Company
  • Completes Application for Collocator Installation Services if they will be performing equipment installation or forwards to company who will be performing the installation work for them.
  • Applicant will provide their past two years financial reports, prior OSHA 200 safety history, credit references, and a list of officers, directors, and/or partners. The information packet will also include an insurance certificate.
  • Returns Application for Collocator Installation Services with all requested documents in information packet to GTE Wholesale Markets Technical Support.
    NOTE: It is the Collocator's responsibility to obtain the application with the other requested documents from their collocator installation company.
Verizon West Wholesale Markets Account Manager
  • Reviews the Application for Collocator Installation Services to ensure it is complete and all requested documents are provided.
  • Notifies Collocator or Collocator's Installation Company of any problems found with documents provided in information packet.
  • Sends Application for Collocator Installation Services with other requested documents to Headquarters Network Services-COEI Support.
Headquarters (HQ) Network Services
  • Reviews Applicant's Application for Collocator Services Request form to ensure that it is complete with all requested documents.
  • Performs a preliminary review of any limitations and/or restrictions noted on the Collocator Information Request form.
  • If preliminary review meets the established requirements, Headquarters Network Services forwards copies of the Collocator Information Request form to Verizon West Safety, Security, and HQ CO/OSP Support Departments to begin investigation of Prospective Collocator
    Installation Company.
SECURITY
  • Investigates the Collocator's financial stability and integrity by:
    • Reviewing the Dun & Bradstreet Report.
    • Requesting a Business Credit Report if a Dun & Bradstreet Report is not available.
    • Requesting their telephone credit history, if a GTE customer.
    • Performing five credit reference checks.
    • Performing a Security Department indices check of the Collocator's officers, directors, and partners.
  • Completes Collocator's installation employee's background investigation checks.
  • Returns the findings of the investigation to Headquarters Network Services within 2 weeks.
Security
  • Reviews the Safety Information provided as part of the Collocator Information Request form identifying any Occupational Safety and Health (OSHA)issues.
  • Returns findings of the review identifying the Collocator as a High-Risk or Low-Risk company to Headquarters Network Services within 2 weeks.
HQ CO/OSP Support
  • Evaluates the Collocator's financial statements for the past 2 years.
  • Sends Financial, Safety, and Security findings to HQ Network Services-COEI Support Manager within 2 weeks requesting approval or disapproval(provide explanation of disapproval)as an authorized COE Installation Collocator.
HQ Network Services-COEI Support Manager
  • Examines the information completed by the HQ CO/OSP Support, Security, and Safety Departments.
  • Authorizes or disapproves adding Collocator to the GTE Authorized Collocators list.
  • Send approval or disapproval e-mail to HQ COE/OSP Support.
HQ Network Services
  • An e-mail notification is sent to Wholesale Markets Account Manager informing them of whether the Collocator has been authorized (approved) or not authorized approved to perform installations of their equipment in Verizon West central offices.
  • If Collocator or Collocator Installation Company is approved, they will be added to Verizon West Authorized Collocator Installation Companies list.
Verizon West Wholesale Markets Account Manager
  • Provided notification to Collocator Installation Company via letter or e-mail that they have been authorized or disapproved to perform installations of Collocator's equipment in Verizon Westcentral offices.
  • Maintains file on all correspondence sent to and received from Collocator or Collocator Installation Company.
