Overview
This form is used to order Directory and Caption Listings.
Based on the type of activity indicated on the local service request
(LSR) not all forms may be required for ordering. To ensure the appropriate
forms are submitted to Verizon, refer to resource information within
Verizon's
Business Rules for determining forms. The Ordering Matrix is found in Section 3.3 within the Appendix of the Order Business Rules.
Description
The DL is sectioned to provide specific information related to the directory listing being requested:
- Administrative—basic account information, order number and telephone number
- Advertising—identifies CLEC advertising contacts and provides yellow page information
- Caption/SLU Indent—contains the level of indent and level status details
- Delivery Address/Information—details for delivery of directory and number of directories
- Listing Control—end user listing details
- Listing Indicators—identifies scoping information, dual name and solicitor information
- Listing Instruction—listing name, telephone number and address
- Remarks—additional information
