Overview
This form is used to order Local Service and contains administrative information, service details for requesting, changing or removing products and services.

Based on the type of activity indicated on the local service request (LSR) not all forms may be required for ordering. To ensure the appropriate forms are submitted to Verizon, refer to resource information within Verizon's Business Rules for determining forms. The Ordering Matrix is found in Section 3.3 within the Appendix of the Order Business Rules.


Description
The LSR is sectioned to provide specific information related to the products and services being requested:
  • Administrative—provides CLEC identification, due date, type of service, and network channel/network channel interface codes
  • Bill—bill location and contact information
  • Contact—CLEC implementation contact and design contact
  • Remarks—additional information
