Order Forms: Loop Service with Number Portability

Overview
This form is used to order Loop Service with Number Portability. Loop Service allows an end user to order a loop that includes all features, functions and capabilities of the transmission facilities and attached electronics. Number Portability allows an end user to retain the same telephone number, regardless of the chosen local service provider. For more details

Based on the type of activity indicated on the local service request
(LSR) not all forms may be required for ordering. To ensure the appropriate
forms are submitted to Verizon, refer to resource information within
Verizon's
Business Rules for determining forms. The Ordering Matrix is found in Section 3.3 within the Appendix of the Order Business Rules.
Description
The LSNP is sectioned to provide specific information related to the loop service with number portability being requested:
- Administrative—basic account information, order number and telephone number
- Remarks—additional information
- Service Details—details including number portability indicators, routing information, channel/pair details, jack information, path details, blocking and transfer call information
