Overview
This form is used to order ISDN BRI/PRI Service and contains administrative information and service details for requesting or changing ISDN BRI/PRI services.
The ISDN BRI/PRI (IS) form is used in conjunction with additional forms for ordering with Verizon.
Based on the type of activity indicated on the (LSR) not all forms may be required for ordering.
To ensure the appropriate forms are submitted to Verizon refer to Section 3.3, the Ordering Matrix, under Related Documents within the business rules for determining forms.
Description
The IS form is sectioned to provide specific information relative to ISDN BRI/PRI:
- Administrative— Provides the quantity of ISDN service being requested
- Service Details— Provides information for the ISDN BRI/PRI services being requested
Verizon provides documentation that explains how to correctly populate the order form and the associated fields.
