NotE: A valid secure ID is required to access any of the reports provided by Verizon East/West through this function. All the fields on this form must be completed before the process starts. Incomplete forms will be returned and will delay the target date.

Important: Once access has been received to the FTP server, you will automatically begin receiving your Directory LVR's 30 days prior to the TELCO CLOSE DATE. Directory LVR's that are already past the prior 30 days to TELCO book close will not be available on the FTP server. Timelines: Please allow 10 business days to process this request if you already have a valid Secure ID. The process to get a new Secure ID varies from additional 10 to 20 business days depending on the requirements.
