End User Special Access Form

Overview

The End User Special Access form is used to provide end user circuit details for new installations, changes to existing services or disconnects.

Based on the type of activity indicated on the access service request (ASR), not all forms may be used for ordering. Related forms for End User Special Access include Multi EC, SALI and Testing Service Request.

Description

The End User Special Access form is sectioned to provide specific information related to the end user services being requested:

• Administrative - basic account information
• Circuit Detail - specific information related to product being ordered
• Primary Location Details - identifies the primary end of a circuit
• Secondary Location Details - identifies the terminating end of a circuit
• Remarks - optional free form field for use to provide further information
