Ring Form

Overview

The Ring form is used to provide ring details for new installations, changes to existing services or disconnects.

Based on the type of activity indicated on the access service request (ASR), not all forms may be used for ordering. Related forms for the Ring form include Multi EC, SALI and Testing Service Request.

Description

The Ring form is sectioned to provide specific information related to the end user services being requested:

• Administrative - basic account information
• Circuit Detail - specific information related to product being ordered
• Primary Location - identifies the primary end of the circuit
• Secondary Location - identifies the terminating end of the circuit
• Remarks - optional free form field for use to provide further information
