Transport Service Form

Overview

The Transport Service form is used to provide Special Access Transport Service circuit details for special access service, switched access facility or unbundled transport.

Based on the type of activity indicated on the access service request (ASR), not all forms may be used for ordering. Related forms for Transport Service include Multi EC, SALI and Testing Service Request.

Description

The Transport Service form is sectioned to provide specific information related to the end user services being requested:

• Administrative - basic account information
• Circuit Detail - specific information related to product being ordered
• Remarks - optional free form field for use to provide further information
