DexMedia LLC (publisher of Verizon print directories) needs a list of Area Code/Prefixes assigned to your company. (i.e. you have secured these numbers through the North American Numbering Council, NANC, processes to assign to your customers.) This information is critical when Verizon is the directory provider for facility based CLECs or for Resellers who are reselling other than Verizon service in a Verizon directory area. If you are reselling Verizon service it is not necessary for you to complete this form. Idearc will use information you supply as a tool when handling listings for your customers and when determining which directories to supply to them.

The CLEC Area Code/Prefix Form, found on this site, is the form used to provide this information to Idearc.

After submitting your original list, as your company grows and acquires new Area Code/Prefixes that will appear locally in a Verizon Directory, please fill out this form to communicate additional new information to Idearc.

Instruction for filling out the Area Code/Prefix List:

Population of all fields is required.

CLEC Name: Your Company Name

CCNA Code: Customer Carrier Name Abbreviation

Area Code: The Area Code (NPA) associated with the Prefix in the next field. One area code entry per line.

Prefix: The Prefix (NXX) associated with the Area Code in the previous field. One prefix entry per line.

Line Number Range: The range of Line Numbers, within the prefix that your company controls. If all, simply fill in field with a 0000-9999.

Directory Code: The code for the directory area where you will use this NPA/NXX. Please indicate if the Area Code/Prefix spans one or more Verizon directories by filling out the Directory Code column with each directory code you will use in association with this NPA/NXX combination.. Directory Codes can be found under "Directory" on this Web site in the Directory Specifications Report.

Directory Section: Some directories have white pages that are sectionalized by city, town or community name. When a directory is sectionalized, you will find that information in the Directory Specifications Report. This column would be used if the directory has multiple sections and the area code/prefix will be used in only one section. .

State: The state where the Area Code/Prefix will be used.

Carrier Type: Indicates the type of contract the CLEC has with Verizon . The type of contracts are:
