Verizon West
will provide verbally or through
WISE Pre-Order up to 12 telephone numbers (TNs) for the same end user at the same address.
Requests for more than 12 numbers must be sent as a Service Inquiry via WISE.
Verizon West
does not guarantee the pending TN
assignment. The end user must be advised by the CLEC not to use the assigned TN for
advertisement or in print until the number is actually installed.
Pre-ordered telephone numbers will be valid for 15 days. If
no LSR is received within 15 days of getting the telephone number (TN), the TN will be
returned to the number pool.
Verizon West
excludes all Centranet, ISDN, and
unbundled loop circuit assignments from pre-ordering electronic and online contact.
Requests of more than 12 new TN s for the same end
user at the same service address are returned on the LSC or LR.

Vanity Numbers
Verizon West
will attempt to provide, either
verbally or on WISE Pre-Order, numbers for assignment based upon the end user's request.
An end user request for a specific telephone number or for additional searches in excess
of the normal amount are subject to local vanity number General Exchange Tariff conditions
and charges that are applied to the CLEC wholesale bill.

Due Date Assignment
Verizon West
will attempt to provide, either verbally or via WISE Pre-Order, due date
intervals through a labor force
management system.
To retain the scheduling reservation, the LSR must be
submitted within 5 hours of the date and timestamp appearing on the Reservation Summary
screen.
If the Due Date is quoted prior to 12:00 Noon, the LSR must
be received by 5:00 p.m. the same day. If the Due Date is quoted after 12:00 Noon and
before 5:00 p.m., the LSR must be received by 12:00 Noon the following day.
Note: The above times are considered to be in the
time zone of the End User.
CLECs will receive the same due date
intervals as Verizon West retail end users for all resale products. The labor
force management system is a resource data base from which both retail and wholesale due
dates are determined on a demand basis.
Designed Complex Services (Examples: T1, DS0,
DS1, DS3, ISDN - PRI, DCS, DFS, Frame Relay) due date intervals are 15
business days.
