Additional Ring Information (ARI) Form

Overview

The Additional Ring Information (ARI) form is used to provide additional ordering specifications. The Ring and SALI forms may also be associated with the ARI form.

Description

The ARI form is sectioned to provide specific information related to the end user services being requested:

• Administrative - basic account information
• Circuit - identifies the first circuit or segment of circuit
• Primary Location - identifies primary end of circuit
• Secondary Location - identifies termination of circuit
• Remarks - optional free form field for use to provide further information
