Clarification / Notification Request (C/NR) Form

Overview

The Clarification / Notification Request (C/NR) form allows Verizon to notify you of the completion of your ASR and / or to provide status of your ASR.

Description

The C/NR form is sectioned to provide specific information related to the end user services being requested:

• Administrative - basic account information
• C/NR - identifies type of clarification / notification
• Error Detail Section - provides error information
