Network Assignment Information (NAI) Form

Overview

The Network Assignment Information (NAI) is used when the carrier is providing intermediate connecting facility assignments, alternate facility/ACTL and /or drop port equipment assignment information. The NAI form is optional and may be associated the following forms:

• FGA Request
• Trunking Request
• End User Special Access Request
• Ring Request

Description

The NAI form is sectioned to provide specific information related to the end user services being requested:

• Administrative - basic account information
• Alternate Service Detail - identify alternate facility and engineering details
• Circuit Detail - specific information related to product being ordered
