Testing Services Request (TSR) Form

Overview

The Testing Services Request (TSR) form is used to order testing services offered in Access Tariffs and to provide end-to-end testing information.

Description

The TSR form is sectioned to provide specific information related to the end user services being requested:

• Administrative - basic account information
• Testing - identifies type of test being requested
• Remarks - optional free form field for use to provide further information
