Multi Point Service Legs (MSL) Form

Overview

The Multi Point Service Legs (MSL) form is used for new installations, changes to existing services or disconnects.

Based on the type of activity indicated on the access service request (ASR), not all forms may be used for ordering. Related forms for MSL include Multi EC, SALI and Testing Service Request.

Description

The MSL form is sectioned to provide specific information related to the end user services being requested:

• Administrative - basic account information
• Circuit Detail - specific information related to product being ordered
• Remarks - optional free form field for use to provide further information
