If you choose to use another carrier's network, you will need to inform Verizon of your choice of network provider. For Verizon East both parties must complete and forward the Shared Network Agreement contract to your Verizon Account Manager. If you do not have a Verizon Account Manager please use this link to contact our Account Team.

For Verizon West you will use the Lease Arrangement (LA) fields;

LA - Lease Arrangement
LADATED - Date of Lease Arrangement
LANM - Lease Authorization Name.

These fields are found on the Access Service Request (ASR) and must be completed at the time services are ordered. The Access Service Ordering Guide (ASOG) contains the industry’s ASR forms and fields that you will need to place your orders.
