Service Offering: Inside Wire Installation and Repair Service (Maintenance Plan)

Product Overview |
|
Inside Wire Installation and Repair Service is an optional service that Verizon offers in connection with certain lines ordered under a Wholesale Advantage Agreement ("Eligible Wholesale Advantage Lines") and for certain resale lines ("Eligible Resale Lines"). To receive this service on Eligible Wholesale Advantage Lines, a competitive local exchange carrier ("CLEC") must have executed a Wholesale Advantage Agreement and must have executed a Wholesale Advantage Inside Wire Installation and Repair Service Amendment (the "Amendment"). To receive this service on Eligible Resale Lines, a CLEC must meet the foregoing requirements and have also executed a Resale Inside Wire Installation and Repair Service Agreement (the "Resale ISW Agreement"). Inside Wire Installation and Repair Service is not available solely for Eligible Resale Lines.
Inside Wire Installation and Repair Service permits a subscribing CLEC, at its request, to provide installation and repair services on the end user's side of the rate demarcation point. The CLEC will incur time and material charges when the technician performs installation work beyond the rate demarcation point, and will incur a monthly recurring per-line charge ("MRC") for repairs beyond the rate demarcation point. The MRC covers most repair visits, although there are exceptions (as set out more fully in the Amendment and the Resale ISW Agreement) for which time and material charges apply. All applicable time and materials charges begin to accrue once the technician arrives at the work location to begin work, as more fully set forth in the Amendment and the Resale ISW Agreement.

Rates
|
|
Applicable pricing for Inside Wire Installation and Repair Service is explained in the Amendment and the Resale ISW Agreement. For installations and certain repairs not covered by the MRC, a CLEC will be charged for both the time actually spent by the technician to complete the work and the materials used. An end user must contact its local telephone service provider, and not Verizon, for information on labor charges for time and materials. |

All Verizon East due date requests are obtained through the Appointment Scheduling Inquiry Pre-Order transaction. This transaction is used to obtain installation dates to negotiate appointments prior to processing Residential and Business POTS Local Service Requests ("LSRs"). |

All Verizon West due date requests are obtained through the Appointment Scheduling Inquiry Pre-Order transaction. This transaction is used to obtain installation dates to negotiate appointments prior to processing LSRs. |

In order to submit an inside wire jack installation order, a CLEC must complete the "IWJK" field on the LSR. Wiring and jack installation is available on a time and materials basis throughout the Verizon footprint. The end user is required to sign the Work Authorization/Completion Acknowledgement (WA/CA) form, or in some areas known as the Premise Work Invoice ("PWI"), for the installation or repair work completed, but the CLEC is responsible to Verizon for payment of services charged. The end user must contact their local telephone service provider, and not Verizon, for pricing and explanations of the items billed on the WA/CA/PWI form. A CLEC must include the following information on the LSR when placing an Inside Wire Installation and Repair Order:
- A contact name and telephone number for the CLEC customer's location (if the LSR is requesting a dispatch).
- A remark to identify location of the jacks (e.g., "Install two new jacks-1 in kitchen and 1 in family room")

Trouble Administration |
|
Before submitting a trouble ticket for repair, please isolate the trouble condition to the Verizon network or to the end user's premises beyond the rate demarcation point. To report that your jacks are not working you can submit a trouble ticket for repair via EB/LSI-TA/TAXI at Trouble Administration.
Repair ticket work will be done only on existing jacks. Verizon will not perform additional installation work when dispatched on a trouble ticket for repairs. A new LSR must be sent to request new or additional jacks.
A CLEC must submit any ISDN-related trouble tickets into EB/LSI-TA/TAXI via the Telephone Number, and not the circuit ID, to have the ticket properly processed. |
