Order Forms: Resale Private Line

Overview
This form is used to order Resale Private Line. Resale Private Line provides the end user a dedicated private line service. For more details refer to the Wholesale Solutions.
Based on the type of activity indicated on the local service request
(LSR) not all forms may be required for ordering. To ensure the appropriate
forms are submitted to Verizon, refer to resource information within
Verizon's
Business Rules for determining forms. The Ordering Matrix is found in Section 3.3 within the Appendix of the Order Business Rules.
Description
The RPL is sectioned to provide specific information related to the Resale Private Line products and services being requested:
- Administrative—basic account information, order number and telephone number
- Bill—final bill information
- Disconnect Information—provides disconnect ECCKT details
- Primary Location—address and access information for primary location
- Primary Location Service details—service details including jack information and circuit assignment details for primary location
- Remarks—additional comments
- Secondary Location—address and access information for secondary location
- Secondary Location Service Details—service details including jack information and circuit assignment details for secondary location
